Stronghold Assessment — Participant Agreement
By completing this assessment, you acknowledge and agree to the following:
1. Purpose. This assessment is designed to provide insights for coaching purposes. It is not a clinical evaluation, psychological diagnosis, or medical examination. Results are intended to support conversations with your practitioner — not to replace professional mental health care.
2. Confidentiality. Your responses are confidential and shared only with your assigned practitioner. Data is stored securely and is not sold or shared with third parties. Your responses are protected in accordance with our
Privacy Policy.
3. Voluntary Participation. Your participation is entirely voluntary. You may stop at any time. You can resume where you left off using your access code.
4. Mental Health Questions. Some questions ask about mental health, mood, and wellbeing. If any question causes distress, you may skip it or stop. If you are in crisis, please call or text
988 (Suicide & Crisis Lifeline) or text HOME to
741741 (Crisis Text Line).
5. Not a Substitute for Treatment. Results from this assessment do not constitute a diagnosis. If results suggest patterns associated with a mental health condition, your practitioner will discuss appropriate next steps with you, which may include a referral to a licensed mental health professional.
6. Accuracy. Please answer as honestly as possible. There are no right or wrong answers. The value of this assessment depends entirely on the accuracy of your responses.
7. Data Use. Anonymized, aggregated data may be used to improve the Stronghold Assessment System. No personally identifiable information is used in this process. By proceeding, you agree to our
